Physical Exam
You must contact Dr. Scott Erickson at 1-801-294-8288 for complete medical clearance. Get a 9:00am appointment and tell them you are with Renaissance Ranch. Dr. Erickson is located at 440 S. Medical Drive, Bountiful Utah.
The charge is $200.  

Drug Test
You must take a drug test daily at Project Reality located at 150 E. 700 S. in Salt Lake City, Utah. You can reach then at 1-801-364-8080. You need to get a 5 panel 1 and tell them you are with Renaissance Ranch.
The charge is $20.

Background Check
You need to contact the Department of Public Safety - Background Check at 1-801-965-4445 for a background check. They are located at 3888 West 5400 South, Taylorsville, Utah 84118.
The charge is $10.

Daily things to do: (If not in Utah detox and/or on waiting list)
1. Call HR at 801-635-9650 daily.
2. Go to a 12-step meeting and get signed off daily.
3. Random/Daily drug testing daily.
